Montana Resources:
A.W.A.R.E., Inc. (Anaconda) Delivers disability services for Montanans with challenging mental health, emotional and in some instances, physical disability needs who otherwise would be served in a more restrictive setting or perhaps not served at all.
BSW, Inc. (Butte) Provides individual plan coordination, intensive services, social activities, job development, community living, transportation, day services, community employment and community supports to individuals in Butte, Dillon and Anaconda who have vocational handicaps resulting from physical, developmental and/or psychological disabilities.
Child Development Center (Missoula) Provides family education and support, respite and a respite house, evaluation and diagnosis, Part C Early Intervention Family Education & Support, Intensive Family Education & Support, Intensive Supported Living, PASARR/MR Evaluation & Specialized Services.
Choteau Activities is a small, private, non-profit corporation in Choteau which provides work activity, day program, community homes, transitional living, independent living and transportation services to adults with developmental disabilities.
Community Option Resource (COR) Enterprises (Billings) Career Guidance Center provides work opportunities for individuals who have difficulty obtaining or maintaining jobs due to occupational barriers. Services include assessments, training, real-work situations, supported employment, anger management, driver's education, personal and social services, and direct placement services. Also provides supported living and community support services for those in need of in-home support.
Consumer Direct Personal Care (Billings & Missoula offices, but coverage is statewide) allows individuals of any age with health care needs to remain in their homes and communities and to avoid placement in an institution. This program is for individuals who are capable of directing their own personal care services or appointing a Personal Representative to act on their behalf. Program requirements include: eligibility for Montana Medicaid; need for assistance with activities of daily living; need for assistance with instrumental activities of daily living; authorization by a health care professional that assistance is necessary; and completion of an assessment for placement in the program.
Counterpoint Inc. (Livingston) is a private, nonprofit corporation that has provided services to adults with disabilities in Park County since 1976. Counterpoint provides residential, vocational and transportation services to approximately 30 adults with disabilities who meet eligibility for funding through the Developmental Disabilities Program or who are referred by Vocational Rehabilitation.
Developmental Educational Assistance Program (Miles City) is a private not for profit corporation that provides family education and support, residential, and crisis intervention services to families whose children are birth to 18 years of age and adults. Services are provided in 28 eastern Montana counties. The main office is located in Miles City with satellite offices in Glendive, Lame Deer and Colstrip.
Easter Seals/Goodwill, Northern Rocky Mountain (Great Falls) Provides day care and preschool services to children with and without disabilities; serves adults with developmental disabilities, the elderly and adults with medical or physical disabilities; provides clinical services, including case management, home health care, home care, private-duty nursing, psychosocial rehabilitation, mental health counseling and case management and hospice; provides job-training and transitional employment for people with disabilities or other barriers to employment to assist them in finding and keeping a job; and provides free information and referral services, educational programs and durable equipment loans.
Hi-Line Home Programs (Glasgow) Provides adult supported living and community supports.
Job Connection (Billings) Private, non-profit corporation provides supported living services.
Mission Mountain Enterprises (Polson & Ronan) Provides personal and social services, and group home, apartment, roommate, home rental, and home ownership living arrangements for people with disabilities, based on each individual's preferences, abilities and strengths, needs, and life span issues.
Missoula Developmental Services Corporation Operates seven group residences located in typical neighborhoods and offering a variety of experiences and options for residential living. Services are provided 24 hours a day, 7 days a week. MDSC also provides day services, with instructional emphasis on vocational skills, and including communication, survival skills and specialized therapies.
Montana Adaptive Equipment Program offers adaptive equipment services to Montanans with developmental disabilities. Services include an equipment recycle program, equipment loan, purchase of new equipment, and professional evaluations for determination of appropriate adaptive equipment.

Montana Center on Disabilities at Montana State University-Billings collaborates with campus, regional, state and national programs in support of increasing the number of leaders with disabilities. MCD sponsors an annual leadership conference for young Montanans with disabilities.
Montana Department of Commerce: Disabled Accessible Affordable Homeownership helps Montanans with mobility impairments acquire affordable, architecturally accessible homes. To qualify, the eligible homebuyer, spouse, child or parent must have a permanent mobility-related physical disability, an annual income not exceeding $30,000 or total family assets exceeding $50,000, and must be a first time homebuyer or have a non-accessible home purchased prior to the disability. Interest rates vary from 2.75% up to 6%, depending on the buyer's annual income.

Montana Home Choice Coalition is a group of Montana citizens; advocates; disability service providers; federal, state, tribal, and local agencies; the housing finance community; realtors; and the home-building industry working to create better community housing choices for all people with disabilities. A.W.A.R.E., Inc. serves as the Coalition's lead coordinating agency.

Montana Universal Newborn Hearing Screening Program Assures that all infants receive screening for hearing loss before hospital discharge, or by one month of age if the infant is not born in a hospital.
MonTECH: Montana Assistive Technology Program Provides information on assistive technology and an Equipment Loan and Exchange service for durable medical equipment, daily living aids, and state-of-the-art electronic aids.
Opportunity Resources (Missoula) Provides a wide array of supports to people with disabilities, including supported employment & placement, group homes and supported living, social programs for seniors with disabilities, art and recreation opportunities, transportation, individual consultation for services, and case management.
Quality Life Concepts (Great Falls) Serves Cascade, Chouteau, Teton, Pondera, Liberty, Glacier, Hill, Toole, and Blaine Counties. QLF has satellite offices in Conrad, Havre, and Shelby. Group home services are available in Great Falls and Conrad. Supported Living is offered in Great Falls, Conrad and Shelby (and is available in each county served). Community support services are provided to individuals living in family homes, group homes, apartments, and their own homes.
Reach, Inc. (Bozeman) is a private, non-profit organization that serves adults in the Gallatin Valley with various disabilities. Reach provides vocational and residential community supports for people with developmental disabilities and survivors of traumatic brain injuries.
Regional Access Mobility Program (RAMP) of Montana is a nonprofit charitable organization which builds interior and exterior wheelchair ramps and other accommodations for accessibility. Services include installing grab bars, widening doors, installing accessible cabinets, installing stair lifts and elevators, and modifying electrical and plumbing systems.
Region IV Family Outreach (Helena) Provides Family Education & Support services, Federal Part C Early Intervention services, General Fund services, and Family Education and Support- Intensive to 12 Southwestern Montana counties.
Resource, Support & Development, Inc. (RSD) (Billings) operates Billings Training Industries-West, Billings Senior Activity Program, Lake Elmo Group Home and Kathy Group Home in Billings, Recreational Adventures & Vacation Experiences (R.A.V.E.), Beartooth Industries/White Group Home and Cooper Group Home in Red Lodge, Snowy Mountain Industries/Snowy Mountain Home and Snowy Mountain Apartments in Lewistown, and Big Horn Industries/Crawford Group Home in Hardin.
Special K Ranch (near Columbus) Christian residential program serves adults, ages 18 and older, whose primary disability is mental retardation and who may need to live within a supportive community all of their lives. Residents must have at least basic self-help and communication skills. Special K Ranch serves 24 people in six separate residences.
Spring Meadow Resources (Helena) Provides vocational services that result in paid employment at a fair wage or in volunteer positions in the community. SMR manages six adult homes in the Helena area and provides supported living (independent living) services to people who live in their own homes. A senior day program is available for people aging with developmental disabilities. The Creative Activities Program is a component of the Day Programs for all adults with disabilities, along with a blind/low vision component, and the community volunteers component.
Support and Techniques for Empowering People (STEP, in Billings) Non-profit corporation offers home based services to children and adults with developmental disabilities and their families in 11 south central Montana counties. Services range from least to most restrictive for all age groups, and service delivery is always based upon individual strengths, needs and personal choice. Services are funded through state and federal dollars and are offered to eligible persons and families at no cost. Offered to the community at large is the Lifespan Respite Program that serves as a "hub" to link respite workers and resources with any family in need. In addition, STEP facilitates various activities designed to provide the public with general information about disability issues, and opportunities to get involved. Some of these activities include Empathy Training (to teach disability awareness), public presentations and the "Respitality Program".
Westmont Independent Support Services (Helena) Provides vocational, residential, supported living, supported employment, and respite services.
YWCA (Billings) Supported Living Program provides residential services to men and women with developmental disabilities in their own homes and apartments and family homes. Services include assistance with grocery shopping, meal planning, cooking, cleaning, making medical appointments, managing healthcare regimens, participation in leisure, recreational and social activities.

Clearinghouse for the Community Living Exchange Collaborative provides information and training and technical assistance to state and territorial recipients of Systems Change Grants for Community Living.
ABC's of Nursing Home Transition Self-study orientation manual for new transition facilitators who assist in the transition process. Materials were developed with input from Centers for Independent Living and various other organizations. The manual helps new facilitators understand and apply the principles and lists resources found useful by successful transition facilitators.
Aging and Disability Resource Centers: Streamlining Access -- Self-Assessment with State Examples This self-assessment helps ADRCs determine how many hoops a customer has to jump through to receive services. Beginning with the customer's initial call, this assessment outlines several hoops that he or she often has to navigate to access long term care information and services. Current grantees provide examples in this tool for streamlining access.
Consumer-Directed Model Training Manual and Personal Care Attendant Handbook Developed by the Arkansas Independent Choices program, the Consumer-Directed training manual provides ideas and suggestions on hiring, training, and managing a personal care attendant. The Personal Care Attendant handbook provides training material to help personal care attendants do their jobs. Both manuals address hiring, communication and management issues.
Nursing Home to Community Program: A Discharge Planning Manual Designed to orient long-term care providers to the nursing home discharge process. Defines roles of the key partners who are involved in returning residents to community living and documents a standard protocol for accessing and maximizing partner resources.
Reaching Out Curriculum West Virginia Real Choice Systems Change grant created a turn-key curriculum to promote inclusive community support for West Virginians of all ages and all disability types. Accessible curriculum addresses advocacy, community living, self-determination and transition for the public, health care and social service providers, administrators and policymakers.

Moving to Work Demonstration Program allows public housing authorities to design and test ways to promote self-sufficiency among assisted families; achieve programmatic efficiency and reduce costs; and increase housing choice for low-income households.
Department of Housing and Urban Development - People with Disabilities Section 504 of the Rehabilitation Act of 1973 (Section 504) prohibits discrimination on the basis of disability in programs, services and activities that receive federal financial assistance. Web site provides resources that inform persons with disabilities about their rights under Section 504 and related statutes, the exercise of those rights, and information and resources available from HUD. Easy Living Home Voluntary program encourages easy access to homes for everyone through a unique certification program that brings builders and home buyers together with homes that are easy to build, easy to live in and easy to sell.
Eldercare Locator provides home and community-based services to hard-to-reach older persons. Among these services are nutrition, such as home-delivered meals or meals served in congregate settings, transportation, legal assistance and health promotion counseling and training. This National Family Caregiver Support Program, focuses on those caring for family members who are chronically ill or who have disabilities. The program also helps those who are caring for younger family members, such as grandchildren and those with mental retardation or developmental disabilities.

Federal Transit Administration Ensures the availability of a transportation system that is accessible, integrated, and efficient, offers flexibility of choices to riders, and enhances community living. Improves services for low-income, elderly and other transit-dependent travelers including people with disabilities.
Easter Seals Project ACTION Federally-funded national technical assistance project matching the interests of public transportation and people with disabilities. Support is provided by a cooperative agreement with the U.S. Department of Transportation, Federal Transit Administration.
National Center for Senior Transportation is administered by Easter Seals and National Association of Area Agencies on Aging. Provides resources for aging/human service providers; transportation providers; and for older adults & caregivers.

United We Ride is a program of the Federal Coordinating Council on Access and Mobility (CCAM), an interdepartmental council that coordinates a variety of federal programs funding transportation for older Americans, people with disabilities and persons with lower incomes. CCAM’s United We Ride Internet website includes information on transportation-focused grant opportunities, state activities, resources, and strategies for coordinating transportation across agencies. A newsletter is also available.
Universal Design Alliance is a non-profit corporation committed to creating awareness and expanding the knowledge of universal design for all ages, sizes, and abilities to designers, builders, and consumers through educational programs, services, and resources. Members include design professionals, students, businesses, and consumers who want to share and learn about universal design.
