Five intermediate outcome goals feed into the long term outcome: <\/span><\/p>\n<p><span style=\"font-family: Arial;\">1. Build capacity of the MTDH program and partnerships;<br \/>\n2. Support direct services and programs that meet the specific needs of people with disabilities;<br \/>\n3. Increase access to generic services, ensuring civil rights of people with disabilities;<br \/>\n4. Improve access to community environments, ensuring civil rights of people with disabilities; and<br \/>\n5. Integrate disability and health agenda into public policies that influence the health of people with disabilities.\u00a0 <\/span><\/p>\n<p><span style=\"font-family: Arial;\">Short-term outcome goals support each of the intermediate outcome goals:\u00a0 <\/span><\/p>\n<p><span style=\"font-family: Arial;\">1. To build capacity: Increase availability of disability and health data; educate partners about disability and health issues, and secure additional funding.<br \/>\n2. To support direct services and programs: Train partners to implement programs and provide services (such as Living Well with a Disability); and support peer mentoring programs (such as Have Healthy Teeth).<br \/>\n3. To increase access to generic services:\u00a0 Increase awareness of public health partners about barriers experienced by people with disabilities; increase awareness of people with disabilities of the benefits of generic services; and support removal of barriers.<br \/>\n4. To improve access to community environments: Increase community awareness of barriers experienced by people with disabilities and support removal of barriers.<br \/>\n5. To integrate the disability and health agenda into public policies: Educate policy professionals, partner with other agencies and programs; people with disabilities must participate in advisory groups; and we will integrate the disability and health agenda into long-range plans.\u00a0 <\/span><\/p>\n<p><span style=\"font-family: Arial;\">Outputs, products and activities support each of the short term outcome goals:<br \/>\n<\/span><\/p>\n<p><span style=\"font-family: Arial;\">1. Surveillance, disability advisors, epidemiology studies and new partnerships contribute to availability of data, education and additional funding.<br \/>\n2. Nutrition, oral health, funding and the Living Well with a Disability Program contribute to training and supporting programs, services and peer mentoring.<br \/>\n3. Assessment tools, curriculum, information and materials, technical assistance, Disability Advisors, and awareness activities contribute to awareness of public health partners and people with disabilities, and contribute to removal of barriers.<br \/>\n4. Surveys, training, the Accessibility Ambassador Program and architectural design increase community awareness of barriers and support their removal.<br \/>\n5.
